Description

Ohel is seeking a Program Manager to oversee our Living in Place case management program for Older Adults diagnosed with severe and persistent mental illness. Located in Staten Island and Brooklyn, the Program Manager will supervise staff as well as maintain a small case load of older adults who are aging in place.

 Responsibilities: 

  • Provide individual and group staff supervision
  • Oversee data collection and entry into program portal
  • Perform in-home assessments and reassessments of homebound older adults and develop comprehensive care plans 
  • Provide extensive and ongoing case management services including case assistance, entitlement counseling, referrals, supportive contact, advocacy and crisis intervention

 Required Qualifications:  

  • LMSW/LMHC
  • Strong interpersonal, clinical, organizational skills
  • Experience in working with the older adult population, benefits, entitlements and resource applications

The ideal candidate is a self-starter who can work independently as well as within a team and has excellent follow up skills.  This full time position is based in Brooklyn and Staten Island, requires home visits, and offers a hybrid schedule. Position includes a competitive benefits package, including health insurance, as well as generous paid time off.

Salary: $87,000- $90,000
